(DOWNLOAD, FILL OUT AND EITHER BRING IN TO BOROUGH HALL OR MAIL IN. CERTIFIED COPIES ARE $10 EACH. IF YOU MAIL IN, PLEASE SEND APPROPRIATE FEE ALONG WITH A PHOTOCOPY OF YOU IDENTIFICATION, IE: PHOTO DRIVERS LICENSE)
